Severe Crowding Correction with Extraction & Clear Aligners

A clinical case treated using Elencia Aligners

Overview

Severe dental crowding involving both the maxillary and mandibular arches can require significant space creation and carefully controlled tooth movement.

This case involved a patient presenting with Class I malocclusion and severe crowding in both the upper and lower arches.

Due to the extent of crowding, an extraction-based orthodontic approach was planned using clear aligners. The treatment incorporated sequential tooth movements, with specific staging protocols used to coordinate canine and incisor retraction.

Frog staging was incorporated into the treatment plan to facilitate controlled canine and anterior tooth movement following space creation. Buttons and elastics were also used as auxiliaries to support the planned orthodontic movements.

Patient’s Concern

Severe Crowding Correction Without Extraction with Clear Aligners and Mini Implants

A clinical case treated using Elencia Aligners

Overview

Severe dental crowding can make the teeth difficult to clean, affect smile aesthetics and significantly influence a patient’s confidence, particularly when the smile is an important part of their professional life.

This case involved a young female singer presenting with Class I malocclusion and severe crowding in both the maxillary and mandibular arches. A highly placed canine was also present, contributing to the significant alignment challenge. Patient also had complete deep bite leading to traumatic occlusion.

The patient was apprehensive about dental extractions and strongly preferred a non-extraction treatment approach.

A customized treatment plan using arch expansion followed by progressive alignment with Elencia Aligners was therefore selected to create the required space while avoiding extractions.

Patient’s Concern

The patient was a professional singer and was particularly conscious of the appearance of her smile.

Because of the severe crowding, she felt uncomfortable performing at public events and was hesitant to face large audiences. Her smile had become a significant source of self-consciousness.

She was also scared of dental extractions and wanted to explore a treatment option that could improve her smile without removing teeth.

The treatment therefore needed to address both the significant dental crowding and the patient’s strong preference for a non-extraction approach.

As a singer, she also reported concerns related to the way her teeth affected her speech and phonetics while singing.

Clinical Findings

Clinical Objectives

The treatment was planned to:

Treatment Approach

A non-extraction treatment approach along with Mini implants was planned in accordance with the patient’s clinical requirements and her strong preference to avoid extractions.

The treatment was divided into two key stages.

Stage 1 – Arch Expansion

The initial aligners were planned to progressively expand and coordinate the arches, creating the space required to accommodate the crowded teeth.
This initial phase focused on improving arch form and developing space for subsequent alignment.

Stage 2 – Progressive Alignment and intrusion with Mini implants

Once adequate space was created, the treatment progressed to sequential alignment of the crowded teeth.

The highly placed canine was gradually brought into the arch through carefully staged tooth movements and retraction with two miniscrews in between maxillary  second premolar and molar, while the remaining teeth were progressively coordinated.

A single labially placed miniscrew in midline in maxilla was used to achieve the desired intrusion to level maxillary arch by engaging elastics to palatally place elastic hooks in maxillary aligners.

The entire treatment was digitally planned and delivered through customized Elencia Aligners, providing an aesthetic and removable alternative to conventional fixed appliances.

🦷 Treatment Summary

Clear Aligner Case Overview

Case Type Class I Malocclusion with Severe Crowding
Treatment Duration 13 Months
Total Aligners 51 Aligners
Upper Arch 26 Aligners
Lower Arch 25px Aligners
Extractions Non-Extraction
Treatment Modality Elencia Aligners

Treatment Outcome

Following treatment, there was a visible improvement in:

The severe crowding was managed through a non-extraction approach, beginning with arch expansion followed by progressive alignment and intrusion using Mini Screws.

The treatment was completed in 13 months using 51 aligners — 26 maxillary and 25 mandibular aligners.

Beyond the clinical improvement, the treatment had a significant personal impact on the patient. With her smile transformed, she developed greater confidence in appearing in front of audiences and performing at public events.

As a professional singer, she also experienced an improvement in her perception of speech and phonetics while singing, adding an important functional dimension to the aesthetic improvement.

Key Benefits of This Case

The Elencia Advantage

From Smile Insecurity to Performance Confidence

For patients whose profession involves frequent public interaction, a smile concern can extend beyond aesthetics and directly affect confidence and self-expression.

In this case, the patient was a young singer who felt hesitant about performing and facing audiences because of her severely crowded smile.

Since she was apprehensive about extractions, a carefully planned non-extraction approach was adopted. Arch expansion was used initially to develop space, followed by progressive alignment of the crowded dentition and correction of deep bite using Mini Screws. Canine retraction was also achieved by Mini Screws.

Elencia Aligners provided an aesthetic and removable treatment option while allowing the movements to be digitally staged throughout the treatment.

The result was not only a more aligned smile but also a renewed sense of confidence that allowed the patient to feel more comfortable performing and facing an audience.

Teen Crowding Correction with Clear Aligners

A clinical case treated using Elencia Aligners

Overview

Orthodontic treatment in growing patients requires careful consideration of the eruption status of permanent teeth, available arch space and the timing of active tooth alignment.

This case involved a 12-year-old boy with partially erupted teeth and dental crowding. Since the permanent dentition had not completely erupted, treatment was planned in two phases to accommodate the developing dentition.

Initially, aligners were used as eruption tabs to help guide the erupting teeth toward their planned positions, alongside arch expansion to develop space within the dental arches.

Once the dentition had erupted sufficiently for active alignment, the second phase of treatment focused on aligning the teeth using Elencia Aligners.

The active alignment phase was completed in 12 months using 43 aligners — 24 maxillary and 19 mandibular aligners.

Patient’s Concern

The patient presented with dental crowding during the transition to a more complete permanent dentition.

Several teeth were only partially erupted, making it important to consider the ongoing eruption process before undertaking comprehensive alignment.

The treatment objective was to guide the developing dentition, create adequate arch space and achieve improved alignment as the teeth erupted into the mouth.

Clinical Findings

Clinical Objectives

The treatment was planned to:

Treatment Approach

A two-phase treatment approach was adopted to accommodate the patient’s developing dentition and the different requirements of eruption guidance and active alignment.

Phase 1 – Eruption Guidance and Arch Expansion

During the initial phase, aligners were used as eruption tabs to help accommodate and guide the erupting teeth toward their intended positions. Arch expansion was also incorporated to develop space and improve arch form.

This phase focused on managing the developing dentition rather than attempting comprehensive alignment before the teeth had erupted sufficiently.

Phase 2 – Comprehensive Alignment

After sufficient eruption of the teeth, the treatment progressed to active alignment.

Elencia Aligners were used to progressively correct the crowding and improve the positioning of the erupted teeth. The aligners were digitally staged to deliver the planned movements throughout the alignment phase.

The second phase required 43 aligners, with 24 maxillary and 19 mandibular aligners.

🦷 Treatment Summary

Clear Aligner Case Overview

Case Type Teen Crowding Correction
Treatment Duration 12 Months
Total Aligners 43 Aligners
Upper Arch 24 Aligners
Lower Arch 19 Aligners
Extractions Non-Extraction
Treatment Modality Elencia Aligners

Treatment Outcome

The before-and-after records demonstrate a visible improvement in the alignment of the patient’s teeth and overall smile appearance.
The treatment outcome included:
The two-phase approach allowed eruption guidance and arch expansion to be addressed before progressing to comprehensive alignment.

The active alignment phase was completed using 43 aligners — 24 maxillary and 19 mandibular aligners — over 12 months.

This case highlights the importance of considering eruption status and treatment timing when planning orthodontic care for younger patients.

Key Benefits of This Treatment

The Elencia Advantage

Adapting Treatment to the Developing Dentition

Orthodontic treatment in younger patients is not always a matter of aligning every tooth immediately. When permanent teeth are still erupting, treatment planning needs to accommodate the changing dentition and the space required for newly erupting teeth.

In this case, treatment was divided into two phases. The initial phase used aligners as eruption tabs alongside arch expansion, helping accommodate the eruption process and develop arch space.

Once the teeth had erupted sufficiently, the second phase focused on comprehensive alignment using Elencia Aligners.

This staged approach allowed treatment to be adapted to the patient’s dental development rather than treating the case as a conventional alignment-only situation.

What is Crowding Correction

Crowding correction with clear aligners is an orthodontic treatment approach used to address insufficient space for teeth to fit properly within the dental arch. This lack of space can cause teeth to overlap, twist, or become misaligned. The goal of treatment is to gradually align crowded teeth, improve dental function, and enhance the overall appearance of the smile.

Frequently Asked Questions

What is dental crowding?

Crowding correction with clear aligners can help address dental crowding when there is insufficient space for teeth to align properly. Clear aligners gradually move crowded or rotated teeth toward planned positions based on an individualized treatment plan.

Yes. Clear aligners can effectively treat mild, moderate, and many severe cases of crowding by gradually moving teeth into their ideal positions using customized treatment plans.
Treatment duration depends on the severity of the crowding. Most cases are completed within 6–18 months, although complex cases may require additional refinement aligners.
You may experience mild pressure or discomfort for the first few days after switching to a new set of aligners. This is a normal sign that the teeth are moving.
Many severe crowding cases can now be treated successfully with advanced clear aligner technology, attachments, interproximal reduction (IPR), or other orthodontic auxiliaries. However, some complex cases may still require a combination of treatments.

As an orthodontist , my experience with the templates of the Elencia aligners is quite exceptional and smooth. The templates are very soft , otherwise the stiff templates tend to fracture the attachment and cause discomfort the patient too.

My doctor recommended Elencia Aligners for my treatment, and I’m really happy I chose them. The aligners were comfortable, easy to wear, and the final results look amazing.

I’ve been using Elencia Aligners for a few months now and I’m honestly surprised at how smooth everything has been. I wasn’t sure what to expect but the aligners are super easy to wear. I’m on my fifth set now and I can already see visible changes in how my teeth are aligned. If you're looking for clear aligners that don’t disrupt your routine, this is a great option.

A friend recommended Elencia Aligners and I’m glad I listened. I saw results within 6 weeks. Highly recommended if you are looking for something more simple and effective.
